The recruitment process

  • Role: Home Care

    • JOB DESCRIPTION - CARE ASSISTANT/SUPPORT WORKER

      JOB DESCRIPTION - CARE ASSISTANT/SUPPORT WORKER

      Main purpose of role:
      • To provide practical help and support to service users under indirect supervision of the
      team leader and undertake delegated work.
      • To participate in plans to promote an environment conclusive to a high
      standard of care according to service users’ needs and wishes.
      • You will be required to work in all areas of the First Practice Healthcare services
      including specialist areas with service users who display severe
      challenging behaviour
      Key Accountabilities:
      • To work within agreed standards and Policies of First Practice Healthcare
      This will involve at all times:
      Ensuring that people are treated with dignity and respect
      People having access to age appropriate material and settings
      Client are assisted as required by care plan, where appropriate encourage self care and self
      management
      encouraging self-advocacy amongst service users
      acting as an advocate on behalf of people where and when necessary
      provide support for clients and families to address issues which affect their health and
      well-being
      linking people into ordinary community facilities in order to meet their
      support needs to promote Social Inclusion as required
      maintain confidentiality in line with First Practice Healthcare Procedure
      Specific Responsibilities:
      • To assist the service users in meeting their physical and emotional needs
      • To assist, under supervision, the administration of medication to service
      users
      • To assist in the implementation of service users’ individual plans based on
      a needs-led day opportunities service
      • To communicate effectively with service users, carers and other
      professionals
      • To attend and participate in staff meetings
      • To attend relevant training courses to further personal development and/or
      to improve the service to users
      • To record information as necessary in a clear and precise manner and in
      accordance with guidelines
      • To ensure a healthy and safe environment for service users, other staff and
      themselves.
      Other Duties
      • To maintain personal and professional development to meet the changing
      demands of the job, participate in appropriate training activities
      • To undertake such other duties, training and / or hours of work as may be
      reasonably required and which are consistent with the general level of
      responsibility of this job.
      • To undertake health and safety duties commensurate with the post and/or
      as detailed in the Health and Safety Policy.

  • Team Leader

    • JOB DESCRIPTION : TEAM LEADER

      TEAM LEADER
      Responsible to: The Managing Director
      Quality Assurance Manager / Care Co-ordinator
      SUMMARY OF ROLE
      You’ll lead or supervise a team of care workers to ensure they provide high quality
      care and support.
      Your role includes:
       managing and supporting workers so they can perform their roles safely and
      to the best of their ability
       supporting your manager to run your workplace according to standards and
      legislation
       acting as a positive role model to colleagues
       developing care plans that outline what care and support individuals need
       leading team meetings and doing inductions, supervisions and appraisals
       working in partnership with other professionals to ensure individuals get the
      support they need.
      MAIN RESPONSIBILITES, TASKS & DUTIES
      Leadership
       Provide leadership to the team acting as a positive role model at all times.
       Manage rotas on a daily basis.
       Lead the shift, ensuring staff give support that follows individual’s care plans
      and daily support plans.
       Provide effective supervision to the carers on your team.
       Provide on call cover as arranged by the Manager, working within on call
      guidance.
       Ensure good team working.
       Motivate, support and mentor the carers when needed.
       At each shift ensure carers complete all necessary paperwork to an acceptable
      standard and sign off.
      Support
       Support people with regards to decision making and lifestyle choices enabling
      them to take control of their own lives, increasing independence.
       Ensure all personal care tasks are carried out in line with the needs, wishes
      and preferences of the individual, following the care plan.

      JOB DESCRIPTION – TEAM LEADER
       Support the development of person centred planning, ensuring each
      individual’s plan has meaningful and achievable goals.
       Work as a key-worker/ co key-worker to coordinate and develop the support
      to the individual using Person Centred Plans.
       Contribute to / attend reviews and multi disciplinary meetings.
       Work with individuals to promote healthy life style by liaising with local health
      care professionals e.g. GPs
       Work in partnership with care managers and other professionals to maximise
      quality of life for individuals.
       Develop positive relationships with family and carers.
       Work in line with the company’s Adult Protection and Prevention of Abuse
      Policy and Whistle Blowing Policy to ensure that people are kept safe at all
      times.
       At all times to work within equal opportunities policy and procedure.
       Ensure risk assessments are carried out and actioned as necessary, in line
      with policies and procedures.
       To be flexible and responsive at all times to meet the changing needs of the
      service and the service users.
       Organise and support service user holidays in line with the policy.

      people living in the home.
      Finance
       Support people to manage their resources and finances.
       To ensure that you receive all receipts, change and any accompanying
      paperwork before the end of the shift.
       To give practical support to individuals in line with their care plan and policy.

       Ensure Petty Cash and people’s monies are checked, balanced and recorded
      at the end of your shift.
       Financial discrepancies must be reported to the manager immediately.
       To keep updated with all financial policy and procedures.
      Administration
       You will be required to work flexibly on on-call rota basis to meet the needs of
      the service; this includes weekends, bank holidays.
       Ensure work is recorded accurately and appropriately in compliance with the
      company requirements and that records made and personal information used
      are in compliance
       You are required to attend review meetings and care assessments using your
      car, record and submit your mileage at the end of each month to the account
      officer for reimbursement.
